Authorized Driver of a Personal Vehicle

I am applying to drive my personal vehicle for Embassy Ministry Events.

Authorized Driver of Embassy Vehicles

I am applying to be an Authorized Driver of the Embassy Bus and Embassy Van.

I understand that texting on my cell phones is not allowed while driving. *

I am 21 years old or older. *

As the driver, I will make sure that all passengers are seated and wearing their seat belt whenever the vehicle is moving. *

Young children must ride in child safety seats as required by state/federal regulations. There may not be more passengers in any vehicle than the number of seatbelts in the vehicle. The driver and/or group leader has full authority for keeping order and discipline in the vehicle at all times.

If there is an accident, I will be responsible for filing appropriate police reports and reporting any accident to Embassy leadership as soon as safely possible. *

The understand that the Embassy reserves the right to remove any driver from the approved driver list, at any time. *

While Driving for Embassy ministry events, I agree to follow traffic laws at all times and failure to do so may result in removal of being an Authorized Embassy Driver. *

I understand that smoking is prohibited at all times while driving for Embassy ministry events and in any Embassy vehicles. *
